Checklist of Clinical Requirements
All clinical requirements are at the student’s expense.

This is a checklist for you to use as you prepare your clinical requirements documentation.

Keep all documentation until requested from the program director, unless specified otherwise below.

IMMUNIZATIONS:

Varicella (chicken pox) – Documentation of either:

  • Serologic test positive for varicella antibody/immunity (positive history of chickenpox)  
    • or if necessary, without serologic evidence of immunity or prior vaccine:
  • Two doses of varicella vaccine at least 4 weeks apart

Hepatitis B Immunity Documentations:

All students must have had the Hep B series of vaccinations (3 doses over a 6-month period). 

  • A completed series of Hepatitis-B vaccine
    • or
  • Informed refusal of the vaccine
    • or
  • Has begun the 3 dose series

Titer Test upon Completing Series:  The CDC recommends (and we require) serologic testing (titer). Specifically, we require a quantitative hepatitis B surface antibody (anti-HBs) to document immunity.

If titer test comes back with a protective concentration of less than 10mIU/mL , student needs to be revaccinated (Repeat Series) with 3 doses (over 4-6 months).

Titer Test following completed Repeat Series:   Another serologic test for immunity 1-2 months after the third dose.

Measles (Rubeola) Documentation:  Those born on or after January 1, 1957:

  • Serologic test positive for measles antibody/immunity.
    • or if necessary, without serologic evidence of immunity or prior vaccine:
  • Two doses of MMR vaccine at least 4 weeks apart*

*Combined MMR Vaccine is vaccine of choice if recipients are likely to be susceptible.

Mumps Documentation:  Those born on or after January 1, 1957:

  • Serologic test positive for mumps antibody/immunity.
    • or if necessary, without serologic evidence of immunity or prior vaccine:
  • Two doses of MMR vaccine at least 4 weeks apart*

*Combined MMR Vaccine is vaccine of choice if recipients are likely to be susceptible.

Rubella Documentation:  Those born on or after January 1, 1957:

  • Serologic test positive for rubella antibody/immunity.
    • or if necessary, without serologic evidence of immunity or prior vaccine:
  • Two doses of MMR vaccine at least 4 weeks apart*

*Combined MMR Vaccine is vaccine of choice if recipients are likely to be susceptible.

Tetanus, Diphtheria, Pertussis (TDAP) Documentation: 

  • One TDaP Booster within 10 years.

Meningococcal (Meningitis) Vaccine (MCV4) Documentation:    Mandatory for all new students <22 years of age

Note: It must be administered at least 10 days prior to the start of the semester.

Documentation of either:

  • Initial dose (2-18 yrs. of age) and Booster (5 yrs. or more later)
    • or
  • Initial dose (11-16 yrs. of age) and Booster (16-18 yrs. of age)

Tuberculin Skin Test*: (PPD - TB test) or QuantFERON Gold TB Blood Test or Chest X-ray (required IF skin test is positive) Documentation: 

  • * Must have been tested within past 6 months.

A negative TB skin test within the past 6 months or, in the event of a positive TB skin test, a chest x-ray within the past three (3) years; OR provide evidence of no TB disease per negative result of interferon-gamma release assay blood test (T-Spot or Quantiferon Gold) within twelve (12) months

Influenza: 

All students should receive annual vaccination against influenza.

  • One dose of influenza vaccine annually, live inactivated injectable influenza vaccine IM or intradermal or live attenuated influenza vaccine (LAIV) intranasally (ONLY for non-pregnant healthy HCP aged 49 or younger).

Other Requirements

Background Check: Order your Background check through CastleBranch and complete within 1 week to 30 days prior to the start of your clinical hours. (Results will remain in CastleBranch)

Drug Screen: Order your Drug Screen through CastleBranch and complete within 1 week to 30 days prior to the start of your clinical hours. (Results will remain in CastleBranch)

Physical Exam: Must be current, within 1 year prior to the start of your clinical hours. HITT Health Data Form

 

Additional Information:

  • All clinical requirements are at the student’s expense.
  • All requirements must be completed by the first day of your clinical.
  • Keep all immunizations records and health data form until requested by the program director.
  • The results from the background check and drug screen will remain in CastleBranch.
